Case Management
RN Case Managers will assist you and your physician in coordination of services while you are in the hospital and after discharge.
A RN Case Manager provides:
Discharge Planning
Case Management can help you in planning post-hospital care, such as information about or placement in board and care homes, convalescent care, or senior independent living. If you are in need of equipment or home care services, the staff will assist you with arrangements for your discharge home. The Patient and Family Services Department coordinate some of these services.
Insurance Authorization
Case Managers also assist you with insurance authorization and can answer questions regarding your coverage options. If you have any questions, please call the Case Management Department at (559) 638-8155 ext 2805.
Patient and Family Services
Social work service is an integral part of many services offered by the hospital. Patient and Family Services recognize the impact an illness or injury has on emotions, finances and discharge needs. Because of this, social work services are available to all patients and families.

Crisis Intervention
Medical social workers are available to support you and your family during the acute phases of illness. They are available daily for emotional support, drug and alcohol information, grief counseling and ethical issues.
Information and Referral
Patient and Family Services has an extensive range of knowledge of community resources to assist you when you or a family member return to post-hospital life. Community Resource Guides are available to all patients and families. Patient and Family Services can provide information about the Durable Power of Attorney for Health Care and help you complete the form while at Sierra Kings Health Care District.
Individual and Family Counseling
Social workers are available to provide counseling surrounding the emotional impact and coping with critical, chronic and terminal illness. In order to receive accurate and up-to-date information about the patient, family members can request a family conference with attending doctors, nurses, social workers and other hospital staff who are in direct contact with the care of the patient.
